Team Members Needed:
RN Case Manager
As an RN Case Manager, you will oversee each of your patient's hospice experience. Our full-time case managers have an average caseload of 12 patients at a time. Depending on the prognoses of your patients, you may have the same patients for weeks to months. As a case manager, you will conduct routine and prn visits in your patients' homes. You'll work with our MD and the rest of the IDG team to develop each of your patient's plan of care. This position is M-F from 8 am to 5 pm and allows you to create your own schedule.
Personal Caregivers
We are accepting applications for caregivers to provide personal care to patients who live alone or whose caregivers need extra assistance caring for their loved ones. These positions are hired directly by our patients and their families at our recommendation.

On-Call RN
Our on-call RNs are available to patients and their families for emergent needs afterhours. As an on-call RN, you will triage calls made on weeknights and weekends. You may handle issues such as injuries, pronouncements, unmanaged symptoms, medication questions or emergent sign-ons. Our prn on-call positions allow you to sign up for shifts according to your schedule, with no minimum or maximum requirement.
Respite Caregivers
We are accepting applications for caregivers who would be willing to provide respite hours or days to allow our patients' caregivers to run errands and take some time to take care of themselves. These positions are hired directly by our patients and their families at our recommendation.
